Dermoid cyst -clinical types, clinical feature , differential diagnosis ,treatment

                          DERMOID CYST

  • Dermoid cyst is lined by squamous epithelium containing  desquamated cells
  • Origin :Face is developed from five processes two maxillary, two mandibular and 1 frontonasal process.
  • Dermoid cyst occurs in the line of embryonic fusion of these processes.
STUDENTS CORNER:
lets see how to approach this question better here you go start with a small introduction to the topic, mention few common sites where dermoid cyst can usually occur in order to remember that here is pneumonic "SERF" where S- Sublingual dermoid, E-External angular dermoid, R-Root of nose, F-Forehead,  followed by listing out different clinical type of dermoid cyst, clinical features ,differential diagnosis and end it of with treatment options .

Common sites:

  • Forehead
  • External angular dermoid
  • Root of nose
  • Sublingual dermoid

CLINIAL TYPES OF DERMOID CYST:

1)CONGENITAL /SEQUESTRATIN DERMOID:

  • They occur along the line of embryonic fusion ,due to dermal cells being buried in deeper plane
  • The cells which are sequestrated in the subcutaneous plane proliferate and liquify to form a cyst

2)IMPLANTATION DERMOID:

  • Common in women, tailor, agriculturist who sustain repeated minor sharp injuries
  • In sharp injury ,few epidermal cells gets implanted in subcutaneous plane resulting in implantation dermoid cyst

3)TERATOMATOUS DERMOID CYST:

  • Teratoma is a tumor arising from totipotential cells
  • Contain ectodermal ,endodermal, mesodermal elements -hair, teeth ,cartilage and bone
  • Common sites: ovary, testis ,retroperitoneum

4)TUBULO-EMBRYONIC DERMOID CYST:

  • Arise from ectodermal tube

CLINICAL FEATURE:

  • Resorption and indentation of the bone beneath
  • Painless, slow growing swelling
  • Soft, cystic and fluctuant swelling
  • Underlying bone defect give clue to diagnosis

DIFFERENTIAL DIAGNOSIS:

  • Sebaceous cyst
  • lipoma
  • Bursa

TREATMENT:

  • Excision of cyst under general anesthesia
  • Neurosurgical approach is required.

CONCLUSION:
Vanakam, my dear stencildent family ,hope you all are doing good here is a post on most frequently asked 5 mark question in general medicine DERMOID CYST ,if you would like to know about another important 5 mark question then do click on this link sebaceous-cyst to learn better .
