                            FRENECTOMY

This particular topic on frenectomy on exam point of view is a most possible 5 mark question so in order to achieve maximum possible marks we need to present this answer in an order ,Firstly begin by giving small introduction on what frenectomy is ,how is it different from frenotomy ,where frenectomy is performed followed by how its supposed to be performed apart from exams its definitely must know topic regarding case selection criteria which has a very vital role ,in clinical practice .

INTRODUCTION:

  • Frenum is a fold of mucous membrane that is enclosed with muscle fiber that attaches to lip and cheek at alveolar mucosa gingiva 
  • If frenum to close to marginal gingiva ,it creates tension on frenum that pull gingival margin away from tooth ,conducive to plaque accumulation.

FRENECTOMY:

  • Complete removal of frenum including its attachment to underlying bone and may be required in correction of an abnormal diastema between maxillary central incisor .







FRENOTOMY:

  • Incision of frenum ,create zone of gingiva between gingival margin and frenum .

Area: 

  • Facial surface between maxillary and mandibular central incisor and canine and premolar 







PROCEDURE:

STEP1:Local anesthesia and engage frenum with hemostat

STEP2:Incise along upper surface of hemostat beyond tip

STEP3:Similar incision along undersurface of hemostat

STEP4:Remove triangular resection portion ,expose brush like fibrous attachment 


STEP5:Horizontal incision separating fibers 

STEP6:Suture ,sometimes graft placed at free gingival graft 

STEP7:Clean site stop bleeding 

STEP8:Cover area with dry aluminium foil and apply periodontal pack

STEP9:Remove pack after 2 weeks






CRITERIA FOR SELECTION:

  • Surgical site free of plaque ,calculus and inflammation 
  • Adequate blood supply to donor tissue
  • Anatomy of recipient and donor 
  • Stability of grafted tissue 
  • Minimal trauma to surgical site
