Now lets get inside our content on propionic acid derivative drug-ibuprofen ,acetic acid derivative drug -ketorolac and fenamate derivative mephenamic acid.
PROPIONIC ACID DERIVATIVE :
IBUPROFEN
- Better tolerated alternative to aspirin ,safest NSAID
- Antiplatelet action is short lasting
- Weaker anti-inflammatory drug
USES:
- Analgesic ,anti-pyretic
- Rheumatoid arthritis
- Tooth extraction(aspirin +codeine)
- Soft tissue injury
ADVERSE EFFECTS :
- Nausea
- Vomiting
- CENTRAL NERVOUS SYSTEM: Headache ,dizziness, blurring of vision ,rash ,itching
PHARMACOKINETICS AND INTERACTION:
- ABSORPTION-Well absorbed orally
- DISTRIBUTION-90% plasma protein bound
- METABOLISM-liver
- EXCRETION-urine ,bile
FENATE DERIVATIVE :
MEPHENAMIC ACID
- Analgesic
- Anti-pyretic
- Week anti-inflammatory drug
- Inhibit cyclooxygenase and prostaglandin action
PHARMACOKINETICS:
- ABSORPTION-Oral is slow but almost complete
- DISTRIBUTION-highly bound to plasma
- METABOLISM- partly metabolized
- EXCRETION-urine, bile
ADVERSE EFFECT :
- Diarrhoea
- Epigastric distress
INDICATION:
- Analgesic in muscle ,joint ,soft tissue
ACETIC ACID DERIVATIVE :
KETOROLAC
- Analgesic
- Modest -anti-inflammatory
- Does not react with opioid receptor ,free of side effect
- Inhibit prostaglandin synthesis
USE:
- Postoperative dental pain
- Musculoskeletal pain
PHARMACOKINETICS:
- ABSORPTION-Rapid in oral and intramuscular route
- DISTRIBUTION-plasma bound
- METABOLISM-glucuronidation pathway
- EXCRETION-60% unchanged in urine
ADVERSE EFFECT:
- Nausea
- Abdominal pain
- Ulceration
- loose stool
- Drowsiness
